Canal and River Trust – Incident Investigation

I love the opportunity to really dig down into the detail. Following initial information gathering by CRT, I was asked to complete an in-depth probing investigation to identify the immediate, underlying and root causes and remedial actions required for two separate incidents. I was able to identify some significant underlying and root causes, delivered in a final report with an action plan to CRT.
